Guardian Ad Litem is the agency occupying office space at 14 South Main in Gainesville, Florida. This is to request approval of the Fifth Amendment to the Lease Agreement effective October 1, 2021 and ending September 30, 2022. This amendment is for a period of one (1) year and modification of rent to which all parties have agreed.

Guardian Ad Litem occupies and operates in the office space. A First Amendment was entered into on June 13, 2017; a Second Amendment was entered into on August 14, 2018; a Third Amendment was entered into on September 24, 2019; a Fourth Amendment was entered into on September 22, 2020.
