Prior Board Motions:
On September 28, 2021, the Board asked the Healthcare Advisory Board to explore a multifaceted approach to achieve health equity that brings community partners together to integrate physical and potential financial assets to involve multiple healthcare providers. Also that the board explore issues related to health as well such as transportation, insurance, and other outreach potential.

Fiscal recommendations in the presentation are those of the advisory board only, and NOT staff recommendations. No CHOICES funding is budgeted to come from the General Fund in the FY23 proposed budget.

Background:
The Healthcare Advisory Board created four subcommittees at its November 17, 2021, meeting. The subcommittees are Primary Care, Dental Health, Mental Health, and Outreach and Case Management. Advisory Board members recruited contributors with expertise in the medical and social sciences fields. The subcommittees met throughout January 2022-April 2022. Committees used data from the Community Health Improvement Plan surveys from zip codes 32641 and 32609 to determine needs, gaps, and barriers as expressed by Alachua County residents living in those zip codes as well as provider surveys and expert knowledge.
