Description:

CHOICES Grant Review Committee Funding Recommendations for FY20-22. Recommending allocations totaling $465,000.00 to local agencies to address health disparities and improve access to healthcare for low-income Alachua County residents.

The Board of County Commissioners (BoCC) approved the CHOICES Indigent Surtax Use Plan and CHOICES RFA 20-201, along with delegating authority to the County Manager to appoint CHOICES Grant Review Committee Members at its May 28, 2019 Board Meeting.

Background:

The Department of Community Support Services began accepting online CHOICES applications via ZoomGrants from interested agencies on June 12, 2019. The application period concluded on July 10, 2019. Thirteen applications were submitted with a total request for funds of $1,021,660.00.

 

The County Manager appointed the five member CHOICES Grant Review Committee on July 1, 2019. The Committee convened on July 23, 2019. It conducted individual application reviews and scoring through August 5, 2019. On August 7, 2019 it met to reach a consensus and final funding allocations totaling $465,000.00.​
