Background:
The Department of Community Support Services began accepting online CHOICES applications via ZoomGrants from interested agencies on June 12, 2019. The application period concluded on July 10, 2019. The Grant Review Committee concluded review of applications and on August 7, 2019 it met to reach a consensus and final funding allocations totaling $465,000.00. The Board of County Commissioners approved the funding allocation at its August 27, 2019 meeting. ACORN Clinic's Saving Smiles program was awarded $52,465.00.
The contract includes new language in Section 7. (Modifications) that authorizes staff to accept and approve revisions to the scope of services, budget, and unit cost worksheet provided the changes are consistent with the submitted proposal and in writing.
