Background:

​The Department of Community Support Services began accepting online CHOICES applications via ZoomGrants from interested agencies on June 12, 2019. The application period concluded on July 10, 2019.  The Grant Review Committee concluded review of applications and on August 7, 2019 it met to reach a consensus and final funding allocations totaling $465,000.00.  The Board of County Commissioners approved the funding allocation at its August 27, 2019 meeting. The Florida Department of Health in Alachua County was awarded $65,376.00 for its We Care Network Medical Services Program. The contract includes authority for county staff to accept and approve written requests for revisions to scope of services and unit of service cost provided that agency requested revisions do not include changes to BoCC approved annual funding allocation.
