Background:
The CHOICES Surtax Use Plan for FY20-22 and RFA 20-201 approved at May 28, 2019 BoCC Meeting. Applications for RFA 20-201 were received and reviewed by County Manager appointed CHOICES Grant Review Committee. All recommended allocations were approved, including the award to the University of Florida Board of Trustees, Mobile Outreach Clinic. The agreement contains a provision for county staff to accept and approve revision to unit of service cost and scope of services, provided there is no fiscal impact.
