Background:
The Southwest Medical Clinic opened to the public in March of 2015. In July of 2016 lease agreement 6652 was executed enabling Alachua County to lease this property to the FDOH to operate the Southwest Health Clinic on a continual basis. The Florida Department of Health (FDOH) is the agency operating and leasing the Southwest Health Clinic located at 816 SW 64th Terrace, Gainesville, FL 32607. The clinic offers health and dental services to meet the needs of the community. Lease 9952 and its amendments have been in effect since 2016 but the last available amendment expires 9/30/2022. This new lease allows for current operations to continue working in the same space for another year, from October 1st, 2022 through September 30th, 2023 with the option to renew once for a five-year period.
