Description:
Lutheran Service of Florida, Inc. (LSF) Amendment 104 is a new contract that reduces the Metamorphosis annual budget by $1,919.00. This is a statewide reduction for the Substance Abuse General Revenue and Substance Abuse Prevention funding from the State of Florida. Amendment 104 also includes provisions for Indemnification and ownership of Intellectual Property.
Lutheran Service of Florida, Inc. (LSF) Amendment 105 increases funding for the Metamorphosis Program to $552,770.00, the Mobile Response Team to $343,307.00 and new funding for 988 National Suicide Prevention Lifeline implementation in the amount of $450,081.00.

Waive the County Policy set forth in Resolution 2014-33 (Indemnification) and County Policy regarding ownership of Intellectual Property. 4. Allow staff to continue negotiating with LSF/DCF regarding the Additional Insured clause. 5. Approve Interfund loans in the amount of $125,000.00 for 988 National Suicide Prevention Lifeline and $100,000.00 for the Mobile Response Team.

Prior Board Motions:
The original LSF agreement was approved by the Board on 8/11/2020 and Amendment 100 on 11/2/2020. On 8/11/2020, the Board delegated contract signature authority to the County Manager for amendments that do not increase or decrease the annual grant allocation by more than 10%. The County Manager approved Amendment 101 on 12/15/20 and Amendment 102 on 4/13/21. Amendment 103 was approved by the Board on 6/22/21

Background:
Staff have been in contact with LSF and DCF to request language changes to Amendment 104 regarding Indemnification and Insurance requirements. Page 5 Section #9 of Amendment 104 includes a provision to Indemnify LSF and Page 50, Section 4 includes a provision to name LSF as an Additional Insured. To date, LSF has not made a final decision on our request to strike these sections. Staff are also awating a response from our Insurer.
Metamorphosis is a long-term residential treatment program for adults with substance abuse and/or mental health illness. The facility has a capacity for 21 clients who participate in a variety of treatment activities. The Program receives funding from the State of Florida to support it operations. These funds are also matched with a contribution from the Alachua County General Fund.
The Alachua County Crisis Center is a member of the National Suicide Prevention Lifeline Network and is one of 12 centers in Florida working on the implementation of the 988 National Suicide Prevention Lifeline. Lutheran Services of Florida is providing initial implementation funding through an appropriation from the Department of Children and Families (DCF). We have been told, DCF will be submitting a Legislative Budget Request for continuation funding to support 988 National Suicide Prevention Lifeline.
In addition to operating a suicide hotline, the Crisis Center also provides mobile crisis response services and on-going counseling services. For the Fiscal Year ending September 30, 2021, the Crisis Center responded to 320 requests for mobile response services including 193 individuals under the age of 25.
