Description:
The State of Florida Department of Health (FDOH) is the agency operating and leasing a portion of 224th SE 24th Street in Gainesville, Florida. This is to request approval of the new Lease Agreement effective October 1, 2021 and ending September 30, 2026. This agreement shall supersede the 2001 agreement as amended by amendments one through eleven. This agreement is for a period of five (5) years to which all parties have agreed.

Payments made to the County by the Health Department are a pass-through to cover their portion (61%) of all utilities. The County has use of the Health Departments' services in lieu of building lease payment.

They agreed to pay and assume the cost of 61% of the utility expenses and for all janitorial services described in the agreement. A First Amendment was entered into on September 27,2002; a Second Amendment was entered into on October 2, 2003; a Third Amendment was entered into on February 23, 2005; a Fourth Amendment was entered into on September 25, 2006; a Fifth Amendment was entered into on March 4. 2007; A Sixth Amendment was entered into on October 1, 2007; A Seventh Amendment was entered into on October 14, 2008; an Eight Amendment was entered into on September 22, 2009; a Ninth Amendment was entered into on August 10, 2010; a Tenth Amendment was entered into on September 27, 2011; and an Eleventh Amendment was entered into on June 28, 2016.
