Approve the issuance of Change Order 1 to Purchase Order 22-1451, to Certified Roofing & Construction, in the amount of $12,500.00, for the demolition and replacement of a septic system and a well under the Alachua County CDBG Housing Program, for the Community Support Services Department. The revised Purchase Order Total is $131,500.00.

Prior Board Motions:

July 13, 2021, Board approved the Bid Awards for demolition/replacement of seven (7) owner occupied homes of low income residents.

 

December 8, 2020, Board approved the Updated Housing Assistance Plan (HAP) guidelines for the Community Development Block Grant Program (CDBG).

Background:

The Change Order Request is due to a finding from the Health Department requiring the septic system at certain parcel be disposed of and replaced.   The existing well did not meet code and a new well must be drilled and installed. This Change Order Request is for the amount of $6,300 for disposal and replacement of a septic system and the amount of $6,200 for new well drilling and installation for a total increase in the project of $12,500. The items are code violations and health and safety issues.

 

Original Purchase Order:                                          $119,000.00

Change Order 1:                                                                 $12,500.00

Revised Purchase Order Total:                     $131,500.00
