The terms and conditions for accepting these funds include, but are not limited to:

-                     Submitting required reports to document the projects or activities on which the funds were expended.

-                     Maintain cost documentation in accordance with federal laws.

-                     Follow applicable billing and collections restrictions for patients transported by ambulance as the result of suspected or confirmed coronavirus.

-                     Submit attestation of acceptance of the terms and conditions through the CARES Provider Relief Fund Portal.

The terms and conditions outlined by the Department of Health & Human Services are attached to this agenda item.  As part of this item, it is requested that submission of the acceptance of these terms in the provider portal is delegated to Fire Rescue.
